Sacramental Years: 

Students making sacraments require two additional documents

1) Baptismal Forms. If your child was not baptized at St Michael's and they are making a Sacrament, we need a copy of their baptismal certificate. 

2) Information Sheets. Click on the links below to access and print these documents: 

Grade 2: First Communion Form

Grade 9+: Confirmation Form

Baptismal certificates and information forms can be scanned and emailed to    or   or mailed to the Religious Education Office at 53 Somers Rd. East Longmeadow, MA 01028.
